1. Print out Visa Application Form (Black or Color)
2. Print out Credit Card Authorization Form
3. Fill them out and sign
4. Mail or bring to our office:
a. The Original Passport (signed and have two clear pages for visas)
b. Copy of “Green Card” (if not a US or Canadian citizen)
c. Visa Application Form (make sure it is signed)
d. Credit Card Authorization Form for the amount of service
e. 1 passport style Photo (if you have one)
f. Your invitation letter (We can provide it)
